Annual Statistical Reporting

The Office of the Information and Privacy Commissioner of Ontario’s Annual Statistics Submission Website is open. You may now input your 2018 statistical report!

The deadline for filing your statistical report is Friday, March 1, 2019.

All institutions under the Freedom of Information and Protection of Privacy Act (FIPPA), the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) and/or the Personal Health Information Protection Act (PHIPA) are required by law to submit their statistics, even those that received no freedom of information requests during the 2018 calendar year. If you are both a health information custodian under PHIPA and an institution under FIPPA or MFIPPA, such as a hospital or medical officer of health, you must submit a FIPPA/MFIPPA and a PHIPA report.

All institutions must file reports online. We do not accept statistical reports by fax or email.

Institutions will be reporting:

  • The number of formal (written) requests for access under FIPPA/MFIPPA and/or PHIPA.
  • The number of refusals under FIPPA/MFIPPA and/or PHIPA, the provisions under which the refusal was made, and the number of occasions on which each provision was invoked.
  • The number of uses or purposes for which personal information, including personal health information, was disclosed where the use or purpose was not included in the personal information bank index under FIPPA/MFIPPA or the written public statement under PHIPA.
  • The amount of fees collected and other information indicating the effort to put into practice the purposes of these statutes.


New! Breach statistics (PHIPA)

New this year, health information custodians must submit health information privacy breach statistics. Health information custodians that are also institutions under FIPPA/MFPPA must submit their statistics, even if they experienced no breaches during the 2018 reporting year. It is easy to complete the online questionnaire; and in the cases where there are no breaches to report, it will be done quickly.

You must report all breaches whether or not you were required to notify the IPC at the time they occurred.
